I understand that participation in physical, recreational, instructional, training, and sporting activities at Seams Performance Centre involves inherent and unforeseen risks. I voluntarily choose to participate in and/or use the facility and its equipment and accept the risks reasonably associated with doing so.

I agree to use the facility, equipment, training areas, and amenities in a safe and responsible manner and to follow facility rules and reasonable instructions from Seams Performance Centre staff, coaches, instructors, or representatives.

To the fullest extent permitted by law, I release and discharge Seams Performance Centre, its owners, employees, contractors, coaches, instructors, volunteers, agents, affiliates, landlords, facility operators, and representatives from claims, demands, damages, losses, costs, or expenses arising from or relating to my participation in activities or use of the facility or its equipment, including, to the extent permitted by law, claims arising from negligence. Nothing in this waiver excludes liability that cannot legally be waived.

I understand that Seams Performance Centre is not responsible for lost, stolen, or damaged personal property. In the event of an injury or medical emergency, I authorize Seams Performance Centre personnel to contact emergency medical services when reasonably necessary.

I understand that certain activities may be operated by independent coaches, teams, associations, organizations, instructors, or facility renters. This waiver continues to apply to my presence at and use of Seams Performance Centre while participating in those activities.

If the participant is under 18 years of age, the undersigned parent or legal guardian confirms that they have authority to provide consent on the participant's behalf and consent to the minor's participation and use of the facility.

By submitting this waiver, I confirm that I have read and understand its terms, voluntarily accept the risks associated with participation and facility use, and agree to the release and waiver described above.

This waiver is governed by the laws of the Province of Ontario and the applicable laws of Canada.
